La mayor parte de los nistagmos consecutivos a las afecciones del órgano vestibular periférico tienen un componente rotatorio superpuesto a un nistagmo horizontal o vertical principal, no presentando casi nunca un nistagmo rotatorio puro. Cuando es de pequeña amplitud, el nistagmo rotatorio puede ser indicativo de una lesión bulbar, y los nistagmos rotatorios de mayor amplitud son muchas veces congénitos.
